Arrival/Late policy:
Arrive 15 minutes early. Late arrival is 16 minutes after the booked appointment, Alex Black Aesthetics cannot guarantee service will be performed that day. If you arrive late and still want to be serviced, we will try to perform the service(s) within the remaining amount of time allotted, and you will be charged for the full amount of the service(s) booked.

Scheduling/Rescheduling and Cancellation/No-show policy:
Scheduling Appointments can be done on-line or in person. It requires a debit or credit card on file. By signing this form, I (the client) authorize my debit/credit card to be held on file for future transactions. No charges will be made unless authorized by me and/or if I am in violation of the rescheduling or cancellation policy.

If you need to make changes to the appointment such as rescheduling and/or cancelling your appointment visit our booking site to do so. Be aware that our system will not allow you to make changes if it is less than 24 hours from the set time of your appointment and at this point it will follow our cancellation policy. After the allotted time to make changes on-line is less than 24 hours, call us and leave a voice message as needed. We will return your call.

Any cancellation made less than 24 hours is subject to be charged for the full amount of the service(s) booked and prepaying for the future service(s) in full prior to scheduling.
No call/show appointment will be charged the total amount of service(s) booked.
We at Alex Black Aesthetics reserve the right to refuse scheduling or rescheduling of services to clients. Services are non-transferable. I understand the person making the appointment is the person receiving the services.

Treatment Plan Policy:
I understand and agree to follow treatment plans designed by Alex Black Aesthetics. This aids in the success of corrective skin care and comes with obligatory conditions such as recurring payments and appointments. Sometimes the frequency ranges from weekly to monthly and daily usage of skin care products at home.

Products Policy:
I understand and agree that corrective skin care treatments performed by Alex Black Aesthetics for skin conditions such as hyperpigmentation and acne are required to use specific products we retail. Products are sold separately. All sales on skin care products are final and no returns or exchanges will be made.

Allergy Policy:
In the event of an allergic reaction, notify us, please see your physician immediately. We do not offer refunds. Potential reactions are always at risk and can occur at any time.

Children Policy:
No children allowed in the treatment room due to the liability concerns and the nature of our service, we do not allow children or any other guest in the treatment room during service. Please arrange for appropriate childcare and additional guests can be seated in the lobby or their vehicles.

Payment Policy:
Payment is due at the time of service. We do not accept checks. Invoice payment is due by the allotted time stated on the invoice. If there is an outstanding or past due balance on an invoice it must be paid in full before services can resume.

Photo/Video Policy:
I understand, any photo(s) and/or video(s) that are taken may be used for promotional and digital marketing. By checking this release, I authorize Alex Black Aesthetics LLC to publish photos and videos of me as it relates to skincare and hair removal services. If you do not want your picture taken, please let us know.

Refund policy:
All sales on skin care products are final.
We do not offer refunds in the event of an allergic reaction.
